If you dally in destroying the phylactery, the lich will return to life at full health about 30 seconds after its death. Even if you manage to destroy the phylactery, the lich can still decay before you get a chance to loot it if you took too long.

A Lich can be found in the Scatter in the Rift. This is a highly intelligent spell caster who went to great lengths to gain power and longevity, at the cost of his soul.
