CHE reports and forms are submitted via several google-form documents. Please use the correct form for what you are reporting.

• Post-Event Reports

  • Complete these after every influence-qualifying event if you want credit. Must be submitted within 3 days after the event.
• Quarterly Reports

  • Reports are due by the 7th of the month following the close of the quarter (April 7/July 7/Oct 7/Jan 7).
• Event Support Requests
  • Link:
  • Requests are due by the 3rd of the month PRIOR to the month that you're requesting support. For example, March event support is due no later than February 3rd. PLEASE NOTE: Requests received after the due date will not be accepted without direct approval from the CHE controller.
