The Faithful Hound is a permanent quest available via the QUEST verb.

Details
While out hunting, randomly a ghostly hound will come into view. At this point you're given the quest The Faithful Hound.
You have received a quest offer for The Faithful Hound.
You have two hours to QUEST ACCEPT.
Once you have accepted the quest you're goal is continue to hunt and search creatures in the specified area and pay attention to any indicators that the hound gives you.
Wraiths will attack you and after you'd dispatch them and search them, the hound will eventually find the one he's looking for and you will continue the quest when notified.
You begin to search the wraith, but are interrupted by the hound nosing around in the remains.

The wraith shrieks in agony, its dark form convulsing as cracks of brilliant light split through the corruption! The shadowy shell shatters apart, revealing the ghost of an old man with kind, bewildered eyes. A spectral hound freezes, recognition flooding his face, then bounds forward with a joyful howl!
Before they can reunite, a ghostly portal swirls open behind the old man! Spectral chains lash out and drag him through as he cries out in terror. Without hesitation, the hound launches himself into the closing rift, vanishing after his owner before the rift slams shut!
