First, a note about the schedule for this. As the form is being made available late, this contest will run through the end of OCTOBER.

This will be the final Premium Contest to follow this format. Once this has concluded and winners have been announced, we will be moving to a straight quarterly raffle format for these prizes. As such, we will have raffles available in December (for the fourth quarter of this year), and announcements will be sent well ahead of time for those.

That said, this time around you get to design a personal heirloom! Please fill out the form (https://forms.gle/uiNjxi2hEbu4zD7S6) in its entirety in order to submit your entry. This is intended to be a scripted item that holds some personal significance, but feel free to get creative. Winners will be chosen in the first couple of days of November.
